Coffee County Schools offers term life insurance and Accidental Death & Dismemberment (AD&D) for employees, spouses, and children through Unum.  Consider your current coverage and whether it would provide adequate protection for your family in the event of your death. These plans also include AD&D coverage.

Voluntary Life and AD&D Coverage Options

You may elect voluntary life insurance for yourself and your dependents through payroll deduction.  

Employee: $10,000 increments up to a maximum of the lesser of 5 times your annual earnings or $500,000

Spouse: $5,000 increments up to a maximum of $250,000

  • Cannot exceed 100% of the employee amount

Child(ren): $10,000 or $20,000

Evidence of Insurability & Medical Underwriting


As a new hire, you may elect up to the Guarantee Issue of $200,000 for yourself, $50,000 for your spouse, and $20,000 for your child(ren) with no health questions. Should you wish to elect an amount that exceeds the Guarantee Issue, an Evidence of Insurability Form is required. You may obtain an EOI Form by calling the Benefits Service Center. To apply, simply complete the form and submit it to Unum for review. You will not be deducted for your pending amount unless / until you are approved.

Important Note: If you elect the minimum amount of coverage as a new hire, you may increase up to the guarantee issue amount at future Open Enrollments. Now is your opportunity to enroll with no health questions.

Voluntary Life Insurance Policy Provisions


  • There is no age cap to be eligible to enroll
  • Spouse life cannot exceed 100% of employee amount
  • Rates are age banded and spouse rates are based on employee's age
  • Child(ren) can be covered until age 26
  • Accidental Death and Dismemberment (AD&D) coverage is included
  • Benefit is doubled in the event of death due to accident
  • Dismemberment benefit is paid according to a schedule outlined in the certificate
  • If you are married to another Coffee County Schools employee:
    • Duplicate coverage is not allowed

Beneficiary Information


Your beneficiary is the person(s) who will receive your life insurance benefits in the event of your death. Your beneficiary can be one person or multiple people, charitable institutions, or your estate. Once named, your beneficiary remains on file until you make a change. If your family situation changes, please review your beneficiary on file and make updates if needed. If you don’t name a beneficiary, your life insurance benefits will automatically go to your estate. You are required to designate your beneficiary during your enrollment, but you can change it at any time.

Portability and Conversion


You have the opportunity to continue your life insurance benefit should you separate employment with Coffee County Schools if certain conditions are met and if application and premium payment is made within 31 days of your coverage end date.  You may either port your policy to an individual term life plan or convert your policy to a whole life plan upon separation.  Additional information regarding these two options may be obtained by contacting Unum.
